Health Services

Health Services at The Academy of the Holy Cross are provided by a Registered Nurse who works with students, parents, other health professionals-- including our Certified Athletic Trainer-- and school staff to promote optimum health and wellness to ensure the student’s academic success.

There is a strong relationship between academic achievement and a student's physical, emotional, and mental health. This link is the foundation for providing our school's Health Services as an important component of our school program. Our school's Health Services provides primary prevention aimed at keeping students in school through appropriate screenings, early identification of students at risk for physical, emotional, and mental health concerns, and case management of students with chronic health concerns. 

FRESHMEN & TRANSFER STUDENT FORMS
DUE TO HEALTH ROOM BY AUGUST 1

A complete immunization record and annual physical must be on file for all incoming freshman and transfer students.  These forms must be signed by a physician.  All medications, including over the counter, require a MD State medication form signed by the authorized prescriber and the parent.  The Health Room does not stock over the counter medication. All medications must be brought from home in the original container and accompanied by the Maryland Medication Administration Authorization From. All student athletes must complete ImPACT Concussion Testing Consent Form and Concussion Information Sheet prior to the first day of practice.
